Administrative Review Update

Two to three weeks prior to your administrative review, an administrative review update must be written

Immediately following your court hearing, this form must be completed

Case Review Form

Prior to your case review, this form must be filled out, and brought to the case review session. Four copies have to be made and distributed at the case review

Court Date Request Form

Used to inquire about a Court Hearing for your CASA child. This should be a last resort after checking with the social worker and the attorneys for the hearing information

Court Memorandum Form The Memo Form is used to update the Court on new information, usually after a court report has been submitted, or at any other time as needed, or if a CASA is fairly new to their case and doesn't have enough information to complete a full court report but does have some information to share with the Court. Court Report Addendum

The Addendum is used when the CASA volunteer wants the Court and all interested parties to be aware of new information after a court report has already been submitted and before the actual hearing takes place

Discovery Request Form

This form must be submitted when a CASA

volunteer would like photocopies of any information in the social worker's file. CFS may refuse to provide copies of certain items, such as a psychological evaluation, but CASAs have a right to most of the

information in the file, such as the court reports.

Each active CASA volunteer must submit a new copy of their driving record to the CASA office once a year.

Group Home Site Info

We are trying to compile information on group homes to help provide our CASA volunteers with some insight into the present placement of their CASA children and be a resource for future placements. If you are able to provide the information, please complete the form and send it to the CASA office. If you have any additional printed material from any group homes please send us that information as well. Once we receive the completed forms, along with any brochures from group homes, we will have a file with information on group homes available for your use in the CASA office.

This form must be filled out once a month along with your Volunteer Hours Log form. It must be mailed (not e-mailed) to your team leader by the 2nd of each month, because of your original signature.

Please note that this form has two pages: on the front is the form itself, and on the back instructions on how to fill out the form. Both pages should be included/printed on one sheet.

This form is used to log activities with your CASA child, continuing education, and other CASA volunteer work. This form may be e-mailed, mailed or faxed to your team leader by the 2nd of each month

60-DAY REVIEW: This form is collected from the Team Leaders every two months to allow them to check in with each of their team members and alert the CASA office of their specific needs. It can be completed by hand (60-days review (hand copy)) or on the computer (60-days review).  Please DO NOT complete the "by hand" form on the computer or the computer form by hand."
